Does this generate a signed change order document?

No, this tracks the log and the cost impact. It does not generate a contract variation.

Why is there a separate "work started" column?

Because it is the expensive question. A pending change your crew is already building is money you are spending with no authority to bill it, and the summary reports exactly that combination on its own line.

How do I record scope the client removed?

As a negative cost impact. It reduces the revised contract value, which a log that only accepts additions could not represent.
